Release of Information
Release of Information form and instructions can be found on the Registrar’s Office SharePoint page and below. The release form must be downloaded and completed then will be uploaded to the secure SharePoint site. The form will not need a physical signature to upload, but you will need your Butler username and password to upload the completed form. It will serve as an electronic signature.
Student Instructions from an Android mobile device
If you do not have a PDF viewer on your mobile device, download Adobe Acrobat Reader for free from the Google Play Store.
From an Android device:
- Go to butlercc.edu/registraruploads
- Log in with your Butler account credentials
- Select Student Resources in the menu
- Select the document called "Information Release.pdf" to open the document
- Select three dots at top left to download
- Select folder to download (usually "downloads" folder)
- Select pencil symbol at bottom right
- Document should open in Adobe (if a sign-on to Adobe/Google/Facebook is requested, this is unnecessary, close by selecting "x" in top right)
- Select Fill and Sign
- Fill in document
- Select pencil icon at bottom to sign
- Select check mark at top left to save
- Upload edited document to Student Uploads for processing:
- Go to Student Uploads
- Select Three Dots (top left)
- Select +New
- Select document type to start Upload
- Enter Student Name, Date of Birth, Phone Number, and "Add attachments" to upload edited document. Select Browse to select the document previously saved on your Android phone.
- Select purple Save button to complete Upload.
Student Instructions from an iPhone or Apple mobile device
If you do not have a PDF viewer on your mobile device, download Adobe Acrobat Reader for free from the iOS App Store.
From an iPhone or Apple device:
- Go to butlercc.edu/registraruploads
- Log in with your Butler account credentials
- Select Student Resources in the menu
- Select the document called "Information Release.pdf" to open the document
- Select three dots at top left to download
- Go to Share Icon (at bottom, square with arrow pointing up)
- Scroll through list, select Import to Acrobat
- From Acrobat screen, choose Fill and Sign option
- Fill out form, select pen at bottom to sign form
- Click Done (blue icon, top left)
- Select "Save on this iPhone"
- Upload edited document to Student Uploads for processing:
- Go to Student Uploads
- Select Three Dots (top left)
- Select +New
- Select document type to start Upload
- Enter Student Name, Date of Birth, Phone Number, and "Add attachments" to upload edited document. Select Browse to select the document previously saved on your iPhone.
- Select purple Save button to complete Upload.
Student Instructions from a computer or laptop
In an internet browser:
- Go to butlercc.edu/registraruploads
- Log in with your Butler account credentials
- Select Student Resources in the left menu
- Select the document called "Information Release.pdf" to open the document
- After document opens in browser, select Download (top toolbar) to edit
- Open downloaded document and make your edits.
- When finished with edits, Download and Save edited document (for your security, do not save your personal data to a public computer).
- Upload edited document to Student Uploads for processing:
- Go to Student Uploads
- Select +New (purple button, top left) and select document name to start Upload
- Enter Student Name, Date of Birth, Phone Number, and "Add attachments" to upload edited document. Select purple Save button to complete Upload.

BETA, Butler's Business Education and Training Analysis group, works with area apprenticeships to help round out educational offerings for their skilled employees. The result is a two-year degree from Butler Community College with credit from your apprenticeship.
Our partnerships are part of the college's Corporate Studies program. Butler was one of the first in the state to offer an Associate Degree in Corporate Studies to assist trade school participants and apprentices in obtaining their two-year degree. This degree program is unique as it can be tailored to any industry.
If you are interested in an apprenticeship program or are a union trades worker, please work with your apprenticeship first to get started. Then, complete a free application for admission. There is no application fee. An advisor will assist you every step of the way.
